Club Committee Notification form

Notification of club committee and office holders must be submitted to MSNSW as soon as practicable after the club’s Annual General Meeting, by submitting the online form below.

Contact details may be circulated to other member clubs or potential members.  Other office holder details are required for contact between those officers and the MSNSW Board, club development project team, Coaching Director and/or Branch Recorders. Please provide a daytime phone number.

If all details are not on hand, you may click “Save & Continue Later” at the bottom of the form, and a link will be emailed to you to enable you to complete the form within 30 days.

Note, if any positions with “required” fields are unfilled or do not apply to your club, please enter “TBA” or “N/A”, and the club contact email address in the email field.
